Overview

A brain lesion refers to a structural abnormality or damage to brain tissue, caused by various factors such as trauma, stroke, tumor, infection, and degenerative disease. Because the brain is the central organ that regulates all body functions, various functional impairments—including motor, sensory, cognitive, linguistic, and emotional disturbances—can appear depending on the location and extent of the damage. Brain lesions may cause temporary or permanent disability, making early diagnosis, appropriate treatment, and rehabilitation important.

Causes and Classification

Brain lesions can be broadly divided into congenital and acquired types according to the time of onset and cause. Congenital brain lesions arise from prenatal hypoxia, infection, genetic factors, metabolic disorders, and the like, with cerebral palsy being a representative example. Acquired brain lesions occur after adulthood and typically include stroke (cerebral infarction, cerebral hemorrhage), traumatic brain injury, brain tumor, brain infection (encephalitis, meningitis), exposure to toxic substances, and degenerative neurological diseases (Alzheimer's disease, Parkinson's disease).

Symptoms

Symptoms are determined by the location of the lesion. For example, frontal lobe damage can cause personality changes and executive dysfunction, parietal lobe damage can lead to sensory loss and spatial perception abnormalities, temporal lobe damage can result in language disorders and memory impairment, and occipital lobe damage may cause visual deficits. In addition, damage to the motor pathways can cause hemiplegia or quadriplegia, and brainstem damage can affect life-sustaining functions such as breathing and heartbeat.

Diagnosis

Neurological examinations and imaging studies are used for diagnosis. Computed tomography (CT) is useful for rapidly identifying hemorrhage or large lesions, while magnetic resonance imaging (MRI) reveals detailed structures such as cerebral infarction, tumors, and demyelinating lesions. In addition, blood tests, electroencephalography (EEG), and neuropsychological tests help assess the functional impact of the lesion.

Treatment and Rehabilitation

Treatment varies according to the underlying disease. In the case of stroke, thrombolytic agents or endovascular procedures are considered, and surgical treatment is considered for hemorrhage. Afterwards, intensive rehabilitation therapy that leverages neuroplasticity is essential. Physical therapy, occupational therapy, speech therapy, and cognitive rehabilitation are used to promote functional recovery, and medication and assistive devices are used when necessary. Recently, new techniques such as robot-assisted rehabilitation therapy and brain stimulation (transcranial magnetic stimulation, transcranial direct current stimulation) are being introduced.

Latest Trends

From 2024 to 2025, artificial intelligence (AI) technology is being actively introduced in the diagnosis and treatment of brain lesions. AI-based image analysis shows high accuracy in the automatic detection and classification of brain lesions, reducing radiologists' reading burden and contributing to early detection. In addition, research into cell and gene therapies is underway, with stem cell therapy aimed at inducing neural regeneration after stroke particularly in clinical trials. Digital therapeutics and virtual reality (VR)-based rehabilitation programs are also being developed to improve access to rehabilitation. Brain-computer interface (BCI) technology is being researched with the goal of allowing patients with severe brain lesions to operate devices using only their thoughts. These new technologies offer new hope for improving the quality of life of brain lesion patients, but cost issues and standardization of clinical application remain as challenges.
